    Good afternoon,

    The local Lions clubs will be performing vision screenings on our elementary school children in late January.

    Does anyone have a standard permission or info document that you send home to parents and guardians prior to the screenings?

    Hi Kara,
    The Lion's club came in to do our districts vision screening in the fall. They provided me with an opt out form that explained how it worked and gave parents the option to opt out if they wanted. You may want to reach out to them to see if your Lion's club has something similar as the one I have lists the De Pere Lions club as that is the city I am in. I will attach it so you can see what it looks like.

    I put communication in our parent newsletter asking parents to notify me if they do not want their child screened.

    Have you seen the links on the Wisconsin Prevent Blindness website? There is an announcement form that we send out.
